STEM OPT Reporting – Data Validation & Self-Evaluation Reporting

Use this form to submit your required STEM OPT data validation reports every six months.

If you have new information to report, you must complete the relevant form within 10 days of the change.

Please check your reporting due date and reporting status on “STEM Student Due Date Announcement” box on your Portal.

If you have just completed 24 months of your STEM extension, you are required to complete the Final Evaluation on Student Progress section on page 5 of the I-983 form located at: https://www.ice.gov/doclib/sevis/pdf/i983.pdf

Once completed, you will need to upload the Final Evaluation on Student Progress at the bottom of this report form before you can submit.

If you have just completed 12 months of your STEM extension, you are required to complete the Evaluation on Student Progress section on page 5 of the I-983 form located at: https://www.ice.gov/doclib/sevis/pdf/i983.pdf

Once completed, you will need to upload the Evaluation on Student Progress at the bottom of this report form before you can submit.
