Information included in this form goes privately to Kim Webb, director of the RSVP Center. This anonymous reporting mechanism is available for survivors, friends, acquaintances or family members of survivors to report incidents of sexual harassment, sexual violence, intimate partner violence and stalking. If you are not the survivor, please get permission to provide information before doing so. This report does not initiate a formal complaint, but rather collects information on incidents of sexual assault, sexual harassment, dating/domestic violence or stalking. Anonymous reports are used to gather information about incidents of sexual assault and violence at Washington University in St. Louis and compiled data are used to inform the community about our campus climate, and to inform educational programming. Statistics will be reflected in our annual security report.

All students are encouraged to consider formally reporting any incidents to WUPD or to the Office of Title IX.
