Housing Maintenance Request Form

Note regarding lock requests: Before you complete this form, we ask that you create a new key card using the kiosk. Often, the issue is that the strip for the key card is scratched up and is responsible for the issue.

Additionally, while filling out this form, we highly recommend you test your card on the door and observe what happens. It is important for you to be as specific as possible; without such details, your door issue may not be resolved. Lastly, please allow 3 business days to pass before you submit a second request regarding your door issue.

In the case your request is an emergency, please contact your RA or RCD by email or phone to make sure your request is handled accordingly. If it is after hours, please contact WUPD at (314) 935-5555.

Note regarding technology requests: If you are requesting a repair for a broken telephone port, cable TV port or network port, please submit your request directly to Student Technology Services.

To complete a maintenance request you may either fill out the online form or print this out and turn it in to the South 40 Office of Residential Life. Please complete all of the spaces below.

If you live in the Washington Avenue Apartments or the 520 Kingsland Avenue Apartments, your maintenance is managed through Quadrangle Housing. Maintenance requests can be made by calling 314-935-9511.
