Use this form if you have approved STEM OPT application and have stopped working.

If you have a pending application and are unemployed, contact your OISS adviser immediately.

Important: even if you have updated your end date in the SEVP Portal, you must still complete this form and the I-983 Evaluation on Student Progress to maintain status.

Before you continue with this form, you must have scanned copies of the following document ready to upload:

  • Completed Evaluation on Student Progress section on page 5 of the I-983 form for your previous employment. The evaluation form is required, even if you have not worked with your employer for 12 months. SEVP Form I-983

Evaluation on Student Progress Upload

To complete your reporting you must now upload the completed Evaluation on Student Progress section on page 5 of the I-983 form for your previous employment. The evaluation form is required, even if you have not worked with your employer for 12 months.
