Request an I-20 Reprint (STEM OPT)

Complete this form if you are on a pending or approved STEM OPT and would like to request a reprint of your I-20. This I-20 will reflect the information you reported on your form I-983.

After you submit this form, an I-20 with a travel signature will be emailed to you within two weeks. As a reminder, travel signatures on STEM OPT must be less than 6 months upon your re-entry into the U.S.
