F-1 OPT Reporting – Leaving the U.S.

This form should ONLY be completed if you are permanently leaving the U.S. and WILL NOT return in F-1 status.

DO NOT complete this form if you are just going on vacation.

Please note that students will only have access to the SEVP Portal for up to six months after their OPT ends. In preparation for account closure, SEVP strongly encourages students to print or save a copy of their portal record for future reference, including their Event History.
